29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Facility, Hammond & Gregg Streets, Carnegie, PA: On or about 08/05/15 no guards were provided on the Wellsaw horizontal band saw to protect the operator from the rotating blade. Approximately 12" of the blade was unguarded. (b) Facility, Hammond & Gregg Streets, Carnegie, PA: On or about 08/06/15 no guard was provided on the Lodge & Shipley 18" lathe to protect the operator from the rotating, protruding clamps (dogs). The dogs extended beyond the chuck approximately 1/2" and no guard was in place to protect the operator when checking or measuring the piece being machined.

29 CFR 1910.179(j)(1)(ii)(a): The employer did not establish frequent (daily to monthly intervals) inspection procedures for cranes in regular service to ensure critical components of the crane are not experiencing excessive wear, deterioration or malfunction: (a) Facility, Hammond & Gregg Streets, Carnegie, PA: On or about 08/05/15 the employer had not established a frequent inspection procedure for the overhead cranes to ensure no defects or deficiencies existed. The employer uses 3, 5, 10 and 25 ton cranes and no frequent inspections are being conducted. Daily inspections of the crane should include the following, but not limited to: functional operating mechanisms for maladjustment, deterioration or leakage in lines, valves, drain pumps and hooks for deformation. In additional, monthly inspections, with signed certification records, should be conducted on the hooks, hoist chains and running ropes.
