29 CFR 1910.23(e)(1): A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfaced throughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, or ramp. The ends of the rails shall not overhang the terminal posts except where such overhang does not constitute a projection hazard. Employees were exposed to fall hazards in that: a) A mid- rail was not in place to keep employees from fall approximately 12 feet 6 inches from the working platform to the concrete below at 1104 Pony Express Hwy Marysville KS. b) In the shipping and receiving area the top rail of the walking platform was only 34 inches in height creating a fall hazard of approximately 6 feet to the concrete below at 1900 North St Marysville KS.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders shall not be stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near any other substance likely to cause or accelerate fire; or in an acetylene generator compartment. Employees were exposed to fire and eruptions of cylinders in that Oxygen cylinder were stored with Acetylene cylinders in the same rack at: a) 1104 Pony Express Hwy, Marysville, KS. b) 1185 Pony Express Hwy, Marysville, KS
