29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(I) through (v)), on all open sides:  In the Service Center Building Department #49, an employee sent to retrieve air filters above a booth was exposed to fall hazards of approximately 11 feet 6 inches from an open sided floor to the concrete below, in that a fall protection system was not in place to keep the employee from falling.

29 CFR 1910.106(b)(2)(vii)(c)(6): The employer did not remove combustible material, empty or full drum or barrel, within the diked area.  Employees working near the diked fueling area were exposed to fire and explosion hazards. The employees were exposed in that flammable liquids (Methyl Ethyl Ketone and Barsol A-2626), empty drums, used oils, and used coolants were stored inside the diked area.

29 CFR 1910.176(b): Storage was not secure. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse:  On or about 03/02/17 at the location of 1900 North St, in Marysville KS., in department #7 at the KOMO B Milling Machine, an employee engaged in metal work was exposed to amputation hazards in that a bundle of square tubing weighing approximately 49.6lbs each was not secured to keep them from sliding and falling.
