29 CFR 1926. 501(a)(2): The employer shall determine if the walking/working surfaces on which its employees are to work have the strength and structural integrity to support employees safely. Employees shall be allowed to work on those surfaces only when the surfaces have the requisite strength and structural integrity:    Employees were exposed to floor collapse hazard at the construction site where a  inch thick cement board subflooring was being used as the working floor.  A section of cement board measuring 2ft by 3ft broke under an employee who then fell 10 feet into the basement. The floor was designed to have  inch plywood installed over the cement board but the plywood had not been installed yet so the floor had limited ability to withstand impact loads or point loads.  Additionally, the following conditions were observed:     a)  The joist floor beams underneath the cement board where the floor broke were spaced 26 inches on-center which did not conform to the project design plan which specified 19 inch spacing; the cement board manufacturer specified 24 inch maximum joist spacing.    b)  The cement board that was used was rated 76 psf (pounds per square foot)  load capacity which did not conform to the project design plan which specified USG or equivalent cement board which has 110 lb psf load capacity.     c)  Sheets of plywood were not placed over the cement board floor in high traffic areas to protect from impact loads, as instructed by the cement board manufacturer (not to be confused with the  inch plywood that was to be installed as part of the completed floor). 29 CFR 1926.502(i)(4): All floor hole covers were not color coded or marked with the word HOLE or COVER to provide warning of the hazard:    a)  The employer placed pieces of particle board over where there were holes in the cement board floors. The covers were not marked "FLOOR HOLE".  The covers were made of 3/8 to 1/2 inch thick particle board and can break if overloaded by heavy construction materials or equipment such as ciinderblocks and pipe frame scaffolds that were being used at the jobsite.
