29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about August 30, 2012, the employer failed to ensure that the procedures used by employees performing lock out tag out on equipment such as, but not limited to Wing Segmenter 1, Wing Segmenter 2, Skinner 1, Skinner 2 and Marel for butterflies machinery had specific procedures for where to place the isolating device.

29 CFR 1910.147(d)(1): The authorized employee did not have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy before the authorized or affected employee turned off: On or about August 30, 2012, the employer failed to ensure that employees performing lock out tag out on equipment such as, but not limited to Wing Segmenter 1, Wing Segmenter 2, Skinner 1, Skinner 2 and Marel for butterflies machinery were protected from unknown types of hazardous energies.
