29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. a. At the site, the employer did not ensure that the yard truck 6029 was taken out of service when the brakes were in need of repair. On or about September 23, 2013 an employee received disabling injuries when they were crushed by the wheels of the yard truck 6029 when disconnecting a trailer.

29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Located at Case Farms in Winesburg, Ohio: a. On the 2013 OSHA 300 log the employer did not ensure that all the require information was filled out in column F. 1. Case number 13-21, CM on 7/22/13, did not include the acutal injury to the foot. 2. Case number 13-23, JD on 8/13/13, did not include the actual injury to the back. 3. Case number 13-24, MA on 8/29/13, did not include the actual injury to the arms and neck. Case Farms was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.29 (b)(1), which was contained in OSHA inspection number 98699, citation number 1, item number 5c and was affirmed as a final order on 12/12/11, with respect to a workplace located at 1818 County Rd 160 Winesburg, Ohio 44690.
