29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:      a)  On or about August 15, 2012, and at times prior, employees were exposed to head injuries as they worked without head protection in areas where shingles and other roofing waste were being thrown from upper levels.
