OSH ACT of 1970 Section (5)(a)(1):     Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards.     (a)  In Bay 3, the employer did not ensure a defective fiberglass stepladder was removed from service. A six-foot Werner step ladder with a bent and cracked rear brace was in service and used to enter and exit vessels. Employees were exposed to fall hazards.

29 CFR 1910.305(j)(1)(i):     Fixtures, lampholders, lamps, rosettes, and receptacles located had live parts normally exposed to employee contact:    (a)  In the Supervisor's Office, an energized outlet approximately 60 inches off the floor was missing a cover plate, exposing employees to electrical hazards.

29 CFR 1910.184(i)(1):     Synthetic web sling(s) were not marked or coded to show rated capacities for each type of hitch and type of synthetic web material:    (a)  In Bay 2,  the DD Sling & Supply synthetic web sling did not have legible identification tags affixed, which included capacity information. Employees were exposed to struck-by and/or crushing hazards.     (b)  In Bay 2, an unidentifiable synthetic web sling did not have a legible identification tag affixed, which included capacity information.
