29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    a) On or about August 29, 2012, an employeeclimbed out of a JLG scissor lift in order to access a higher level and was exposed to fall hazards in excess of 11 feet. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) 0n or about August 29, 2012, a employee was standing on a horizontal beam trying to access an adjacent area in order to install a screw.  the employee was exposed to fall hazards in excess of 13 feet without adequate fall protection.  ??? 29 CFR 926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a)  On or about August 28, 2012 and at times prior, employees were working in an upright aerial lift that had guard rails in place but employees were not provided with a personal fall arrest system or a personal fall restraint system to protect them from catapult hazards.
