29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    (a)  On or about August 12, 2015, the employer did not ensure employees were protected from shock and electrocutions hazards while cutting the soffit around an energized and exposed light bulb.       Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a)  On or about August 12, 2015, the employer did not protect employees from fall hazards of greater than 9 feet while cutting the edge of a roof.  The workers did not use personal fall arrest systems or have any other methods of fall protection.     Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").    T-Bone Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 615958, citation number 1, item number 1b and was affirmed as a final order on November 21, 2012, with respect to a workplace located at 840 Spanish Bit Road in Colorado Springs, Colorado.
