29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about September 4, 2012, at 5480 Vista Brook Drive, Suwanee, Georgia: An employee engaged in roof repair work was exposed to a fall hazard that ranged from approximately 16.6-feet to 20-feet when the employee installed flashing on a porch roof without using fall protection.

29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level: On or about September 4, 2012, at 5480 Vista Brook Drive, Suwanee, Georgia: An employee engaged in roof repair work was exposed to a fall hazard of approximately 20-feet when the employee was installing roof flashing while standing on an aluminum extension ladder that was set directly on its side rails on penetrable ground.
