29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2): Well site: On or about August 21, 2012, and time prior to, an employee providing mud logging services during the drilling of an oil/gas well used personal protective equipment as, but not limited too, safety glasses, steel toe footwear, and a hard hat. The employer did not have a certification of the personal protective equipment assessment.
