29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a. On or about August 17, 2012, at PX/Commissary Bldg, Fort Buchanan, PR, employees on top of the roof were not provided with a means to access the ladder's access point at 36 in from the roof level. The employees had been using a portable, self-supported ladder to gain access to the roof of building from the ground level. The maximum fall was 24' to the ground level. NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to whichthe ladder was used to gain access: a. On or about August 17, 2012, at the Commisary/PX Bldg., Fort Buchanan, PR, employees had been using a portable ladder which was not extended at least 3 feet to get access to the roof while engaged in roofing operations at heights up to 24 feet from the ground level. NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.
