29 CFR 1926.50(g): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a) At the project site, on or about September 7, 2012, and at times prior thereto, an eye wash for quick drenching or flushing of the eyes was not provided at the work area. An employee was applying Portland cement onto the floor surface without the use of eye protection. Per the MSDS, Immediately flush eye thoroughly with water. Continue flushing eye for at least 15 minutes, including under lids, to remove all particles. Call physician immediately.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    (a)  On September 7, 2012, or at times prior thereto, the employer failed to ensure the use of appropriate gloves while mixing and applying Portland cement.  The Portland cement was mixed with water in a wheelbarrow and applied onto the floor surface.  The exposed employee wore gloves with a latex material on the palm side and a cotton mesh material on the other side instead of gloves made of a butyl or nitrile material.
