29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:      (a) Facility, 376 Industrial Boulevard, Everett, PA:  On or about 09/05/12 the employer had not conducted an annual inspection of the energy control procedure(s) at least annually to ensure the procedure and requirement of the standard were being followed.  The employer had procedures included in the program for equipment that was no longer in service; the employer had made changes to equipment used in the facility and had not updated the energy control procedure to reflect this.

29 CFR 1910.147(c)(7)(iv): The employer did not certify that employee training had been accomplished and kept up to date:    (a) Facility, 376 Industrial Boulevard, Everett, PA:  On or about 09/05/12 the employer did not certify that employee training in energy control had been accomplished and kept up to date.  The employer had made changes to energy control procedures and the employees affected by these changes had not received training to ensure they were kept up to date on how these changes would affect their work.
