29 CFR 1910.178(a)(4): Modifications and additions which affected capacity and safe operation were performed by the customer or user without manufacturers written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly: (a) Nidi Tec Inc at 5686 Logan Ct Unit C, Denver, CO 80216: On and before July 3, 2012, the employer used an attachment for the forklift that affected the capacity and safe operation of the forklift. The attachment did not have a plate, tag or decal which indicated its weight capacity. This condition potentially exposes employees to struck-by and crushing hazards. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1910.184(f)(1): Wire rope sling(s) were used with loads in excess of rated capacities shown in Tables N1843 through N18414: (a) Nidi Tec Inc at 5686 Logan Ct Unit C, Denver, CO 80216: On and before July 3, 2012, the employer used wire rope slings for moving bundles of slabs. The slings did not have tags showing their rated weight capacity. This condition potentially exposes employees to struck-by and crushing hazards. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).
