29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employees working from an aerial lift: (a) Building 3 - An employee used a JLG aerial lift to access the roof of the four-story high building. The employee did not wear a harness and lanyard and attach to the boom or basket of the lift for fall protection, on or about September 10, 2012.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Building 3 - An employee worked, caulking vent pipes and replacing shingles, on the roof of the four-story high building. The roof had slopes of 6 in 12 and 12 in 12. The employee did not use any fall protection while working on the roof, on or about September 10, 2012.
