29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about 9/7/2012 employees at the VA Medical Center located at 4101 4th St. Trafficway, Leavenworth, Kansas were engaged in disassembling a Hydro Mobile scaffold Series 300. During high wind conditions, employees were exposed to hazards associated with scaffold colapse.

29 CFR 1926.451(c)(2)(ii): Unstable objects shall not be used to support scaffolds or platform units. On or about 9/7/12 employees at VA Medical Center located at 4101 4th St. Trafficway, Leavenworth, Kansas, a Hydro Mobile Scaffold collapsed during disassembly. The scaffold was erected on an unstable base (made of pieces loose pieces of lumber) and during scaffold disassembly the base pedistal slid causeing the scaffold to collapse. Employees were exposed to hazards associated with scaffold collapse.
