29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a) Jobsite - 1413 South 19th Street- A ladder or other safe means of access was not provided to employees working on the tubular welded scaffold. Employees were using the side frames and cross bracing to access the working levels. Obsreved on or about 09/11/12.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: a) Jobsite-1413 South 19th Street - The employer did not provide each employee performing stucco operations from a tubular welded scaffold with training to recognize the hazards and to understand the procedures to control the hazards associated with operations, on or about 09/11/12.

Observed on or about 09/11/12. JC CONSTRUCTION WAS PREVIOUSLY CITED FOR THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION #314232984, CITATION 1 ITEM 1a AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 15, 2011 WITH RESPECT TO THE WORKSITE LOCATED AT 1136 S. 23RD STREET, PHILADELPHIA, PA.

Observed on or about 09/11/2012 JC CONSTRUCTION WAS PREVIOUSLY CITED FOR THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION #314232612, CITATION 1 ITEM 3 AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 11, 2011 WITH RESPECT TO THE WORKSITE LOCATED AT 1017 N. 3RD STREET, PHILADELPHIA, PA.
