29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    On September 12, 2012, an employee was not protected from a fall while working from a metal frame scaffold, exposing the employee to a fall hazard of 13.10 feet.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      On September 12, 2012, employees were not protected from a fall while walking/ working from the exterior top plate of a commercial building, exposing employees to a fall hazard of over 18.7 feet.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
