29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. a. On or about February 27, 2017, at the above addressed jobsite, an employee performing carpentry operations from a JLG model 400S telescopic boom aerial lift was not wearing a complete fall arrest system. The employee was thereby exposed to the hazards associated with a fall of approximately fifteen (15') feet.
