29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    A).	On or about 09/10/12, at the site of 61-43 231st Street. Oakland Gardens, NY    Employees working underneath of scaffolds were in danger of head injury from falling objects such as, but not limited to tools and bricks from other employees who on the second section of the scaffolds. Protective helmets were not provided.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

Oakland Gardens, NY    Employee was cutting tiles on the pitch roof with a saw machine without a guard.      ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs g(1)(i) through g(1)(vi) of this section, each employee working on a scaffold more than 10 above a lower level was not protected from falling to that lower level by the use of guardrails or personal fall arrest systems:    	A).	On or about 09/10/12, scaffolds at 61-43 231st Street. Oakland Gardens, NY    Employees bricklaying exterior wall on scaffolds were exposed to the hazard of falling approximately 13 ft to the ground. Guardrails or personal fall arrest systems were not provided.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
