29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports: (a) C.K. Stone. LLC at 2014 Grand Ave., Glenwood Springs, CO 81601: On and before 9/12/12, the exposing employer did not ensure the working platform on the scaffold was fully planked in that only one plank was placed on the scaffold. This condition exposed the employee to a fall of approximately 15 feet. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.451(g)(1): Each employee on a scaffold 10 feet above a lower level was not protected from falling to the lower level: (a) C.K. Stone. LLC at 2014 Grand Ave., Glenwood Springs, CO 81601: On and before 9/12/12, the exposing employer did not ensure that employees were protected from a fall from elevation in that guardrails or other suitable fall protection were not properly installed on the working platform of the scaffolding. This condition exposed employees to a fall of approximate 15 feet. C.K. Stone, LLC was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.451(g)(1) which was contained in OSHA inspection Number 330424, Citation 1 Item 1 issued on 5/17/12, and became final order on 6/4/12, with respect to a workplace located at 119 South Spring Street, Aspen, CO 81611. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet"). Abatement Note: The employees were not protected by guardrails installed in accordance with 29 CFR 1926.451(g)(1). The scaffold was fully braced however, the bracing did not meet the requirements as a partial guardrail as detailed in 29 CFR 1926.451(g)(4)(xv).
