OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a) Associated Thermoforming Inc. at 765 Second St. in Berthoud, CO: the employer did not ensure that flammable chemicals such as MA-300 Adhesive were stored in a laboratory-safe or flammable material refrigerator/freezer. This condition exposed employees to fire and burn hazards.

29 CFR 1910.1200(h)(3): The employee training did not include the requirements of 29 CFR 1910.1200(h)(3)(i) through (h)(3)(iv): (a) Associated Thermoforming, Inc. at 765 N. 2nd St. in Berthoud, CO: Employee training did not include the requirments of paragrahs (h)(3)(i) through h(3)(iv) in that employees working with MA300 Adhesive were not informed of the health hazards of this chemical nor were they informed of the measures to take to protect themselves, such as appropriate work practices, emergency procedures, and personal protective equipment to be used. This condition exposed employees to a skin sensitization hazard due to the methyl methacrylate contained in the adhesive. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
