29 CFR 1910.36(d)(1): The employer did not ensure that exit doors open from the inside without keys, tools, or special knowledge:     (a) Associated Thermoforming, Inc, 765 N. 2nd Street, Berthoud, CO 80513: On and before September 13, 2012 the employer did not ensure that the exit door located in the material/warehouse was equipped with a breakaway bar to facilitate quick unimpeded egress from the building without the use of keys, tools, or special knowledge. This condition limited employee ability to quickly exit the building during an emergency.     (b) Associated Thermoforming, Inc, 765 N. 2nd Street, Berthoud, CO 80513: On and before September 13, 2012 the employer did not ensure that the exit door located in the shipping & receiving room was equipped with a breakaway bar to facilitate quick unimpeded egress from the building without the use of keys, tools, or special knowledge. This condition limited employee ability to quickly exit the building during an emergency.     (c) Associated Thermoforming, Inc, 765 N. 2nd Street, Berthoud, CO 80513: On and before September 13, 2012 the employer did not ensure that the exit door located in the regrind area was equipped with a breakaway bar to facilitate quick unimpeded egress from the building without the use of keys, tools, or special knowledge. The training shall include the following:    (a) Associated Thermoforming, Inc, 765 N. 2nd Street, Berthoud, CO 80513: On and before September 13, 2012, the employer did not provide adequate training to ensure that employees acquired the knowledge and skills necessary for the safe application, usage and removal of energy control devices, including but not limited to the following machines or equipment:    1) 611R Rotary Former  2) Ram Jet Marathon Trash Compactor    This condition exposed employees to amputation hazards.    Associated Thermoforming, Inc was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1910.147(c)(7)(i) which was contained in OSHA Inspection Number 311906440, Citation Number 1, Item 6(a) issued on December 30, 2008 and became Final Order on November 19, 2008 with respect to a workplace located at 765 N. 2nd Street, Berthoud, CO 80513.

One drive shaft roller end was exposed at the location identified as G-27. This condition exposed employees to hazards by contacting shaft end.    (b) Associated Thermoforming, Inc, 765 N. 2nd Street, Berthoud, CO 80513: On and before September 13, 2012,  the employer did not provide guarding to protect employees from hazards created by the ingoing nip points and rotating horizontal shafts on the Motion Master and Royce Routers.  This condition exposed employees to hazards by contacting the moving router table and shaft end.
