At the workplace, a ripsaw near the center of the sawmill became jammed. The employer did not ensure affected employees were removed from the area while lock-out/tag-out of the ripsaw was being performed, exposing employees to an amputation hazard. At the workplace on 9/13/2012, during the inspection the ripsaw became jammed. An affected employee near the ripsaw began to help clear the jam using a piece of wood. The authorized employee was clearing the jam.

At the workplace, the employer did not have a specific procedure to control hazardous energy for machines located in the sawmill to include but not limited to the head saw, planer, lathes and equalizers, exposing employees to amputation hazard(s). Ames True Temper, Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 314243643, citation number 02, item number 01 and was affirmed as a final order on March 7, 2011, with respect to the workplace located at 1 True Temper Drive Carlisle, PA 17015.
