29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a) Mill, on or about 8/9/13: Hawker Dayton dowel machine, lockout procedures were not utilized prior to clearing a jammed dowel from the cutter head and resulted in a finger amputation due to the employee's hand being pulled into the out-feed rollers.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Mill, on or about 8/27/13: Hawker Dayton dowel machine, out-feed rollers were not adequately guarded in that the safety switch connected to the guard could easily be by-passed. An employee received a finger amputation due to by-passing this safety switch.
