29 CFR 1926.502(b)(1):  The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:  (a) Main Roof of the Building - An employee worked from the flat roof, 13 feet 4 inches above the ground, on a roof-top HVAC unit.  The roof had a parapet wall with a height of 20 inches, which did not meet the minimum top edge height requirement of 39 inches for a guardrail system, on or about August 21, 2012.
