29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 9 September, 2012 and times prior at the I.D. Grinders #1 & #2: The employer failed to ensure that the operator's hands were protected from being caught-between the clamp push-rod and rear of cylinder during loading operations. The operators were required to hold the rear of the cylinder for proper alignment of the cylinder into the drum ring while they pushed the clamp extension button, allowing the clamp push-rod to move forward and seat into the rear of the cylinder.
