29 CFR 1926.760(a)(1):     Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    Jobsite located at 2645 City Highway 18, Cedar Bluffs, NE and throughout the company:  On September 12, 2012 seven employees were installing standing seam metal roofing at the job site without fall protection (five were directly exposed to a fall hazard).  Two of those employees fell from the roof when a section of metal decking was picked up by gust of wind which knocked them from the roof.  The employees fell 23-25 feet sustaining serious injuries with one succumbing to those injuries.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

This employer is engaged in the erection of standing seam metal roofing, an activity covered under this Subpart.  On September 12, 2012, two employees sustained serious injuries, one fatal, when they fell from a roof where fall protection was required.  The employees installing a standing seam metal roof had not been trained as outlined under Subpart R.      Abatement certification and abatement documentation is required for this violation.
