29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation: The employer is failing to protect employees from the hazards of continued injuries by not properly identifying injury/illness trends from the OSHA 300 log. This was most recently documented at a worksite located at or near 9000 Andermatt Dr., Lincoln, Nebraska 68526 on or about November 28, 2016. The employer did not ensure that the OSHA Form 300 for the calendar year of 2016 was completed as required by the regulation to include, but not limited to: a) Back and both feet - Object/substance that directly injured or made person ill was not described.
