Employees were exposed to struck-by and caught-in-between hazards.    B.	Griffin Industries, LLC DBA Bakery Feeds, located at 12850 Quarry Rd., North Baltimore, OH 45872: On or about September 13, 2012, the employer did not remove powered industrial vehicle John Deere B517 (Model 544K, S/N 1DW544KHLA0631299) from use based on mechanical defects such as, but not limited to, broken steering column. 29 CFR 1910.23(c)(1):     Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    Griffin Industries, LLC DBA Bakery Feeds, located at 12850 Quarry Rd., North Baltimore, OH 45872: On or about September 13, 2012, the employer did not ensure that employees were properly protected from fall hazards while conducting cleaning activities inside the ductwork located between the dryer drop box and cyclones. Employees were using inadequate fall protection while working inside the ductwork with unprotected sides that are adjacent to cyclone 1 and cyclone 2 in the facility. Employees were exposed to fall hazards of approximately 20 feet from the ductwork's leading edge to the bottom of cyclones.    Griffin Industries, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.23(c)(1)), which was contained in OSHA Inspection # 311820823, Citation number 2,  Item number 1, and was affirmed as a final order on September 30, 2009, with respect to a workplace located at 11313 SE 52nd Street, Starke, FL 32091.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
