29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated. a) On September 14, 2012, at a jobsite located at 6450 Chatham Ave, Kansas City, MO., employees were exposed serious fall hazards, in that employees were observed using an aerial lift for access to elevated work areas without using required fall protection equipment. The employees were exposed to fall hazards of approx. 17 feet. Abatement certification for this violation is not required.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a) On September 14, 2012, at a jobsite located at 6450 Chatham Ave, Kansas City, MO., employees were exposed serious fall hazards, in that employees engaged in steel erection activities to include but not limited to placement of metal decking, were exposed to unprotected sides and leading edges without the use of fall protection equipment or the establishment of a controlled decking zone. The employees were exposed to fall hazards of approx. 17 to 19 feet. Abatement certification for this violation is not required.

Exposed fall hazards were approx. 17 to 19 feet. Abatement certification for this violation is required. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
