29 CFR 1910.1048(m)(5):     The employer did not develop, implement, and maintain at the workplace, a written hazard communication program for formaldehyde exposures in the workplace:    At this facility employees engaged in beauty operator activities, such as, but not limited to, hairstyling, perms, Brazilian Blowouts, relaxers, highlights, creative color, evening up-do styling, feather extensions, flat ironing, and shampoo, condition and blow-dry services were exposed to formaldehyde gas.    The employer has not developed a program which would address labeling and other forms of warning on chemical containers, material safety data sheets and employee information and training on the hazards associated with formaldehyde gas.    The written program must also contain the following:    a.  the methods the employer will use to inform and train employees of the known associated hazards, and    b.  establish procedures to evaluate the effectiveness of the program.

29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    At this facility employees involved in beauty operator and housekeeping activities were exposed to hazardous chemicals, such as, but not limited to, bleach.    The employer has not developed a program which would address labeling and other forms of warning on chemical containers, material safety data sheets and employee information and training on the hazards associated with chemicals utilized at this worksite.    The written program must also contain the following:    a.  a list of all hazardous chemicals onsite,    b.  the methods the employer will use to inform and train employees of the hazards of non-routine tasks and the known associated hazards,    c.  the hazards of chemicals stored onsite, and    d.  establish procedures to evaluate the effectiveness of the program.
