OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:     a)	Stucco Works, Inc. at 2014 Grand Ave., Glenwood Springs, CO 81601, on and before 9/12/12, the employer did not ensure that employee in an elevated personnel platform was protected from falling. The operator left the truck unattended while the employee was in the bucket. This condition exposed employees to fall hazards of approximately 21 feet from the bucket to the ground.     Among other methods, some feasible and acceptable abatement methods include:  1)  The operator of a rough terrain forklift truck is attended when the operator is less than 25 ft (7.6 m) from the truck, which remains in his view which meets the requirements of ANSI B56.6-1992 Section 6.2.10.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a)	Stucco Works, Inc. at 2104 Grand Ave., Glenwood Springs, CO 81601: On and before 9/12/12, the employer did not ensure that each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system.  Employees were plastering the west side and south side of the building located at a parapet measured at 1'-10 inches and 2 feet wide. This condition exposed the employees to a fall hazard of approximately 21 feet.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in the mounting and dismounting the ladder:     (a)	Stucco Works, Inc. at 2014 Grand Ave., Glenwood Springs, CO 81601, on and before 9/12/12, the employer did not ensure that employees were utilizing portable ladders in such a way that enabled them to access and egress from the roof using ladders that extended 3 feet above the walking and working surface. Employees were using a portable ladder for access to the roof. Employees were exposed to a fall while attempting to use the ladder.
