29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about 7/27/2012, an employee was injured operating a RAS TURBObend 61.31 folding machine, located at C1891 Hwy 153 Stratford, WI 54484, when her arms were crushed. The point of operation between the clamping beam and work table was not guarded, exposing other employees to a crushed by hazard during the operating cycle. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
