29 CFR 1926.20(b)(2): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part; such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On August 29, 2019 at 5720 Windy Drive in Stevens Point, the employer did not ensure that a competent person was performing frequent and regular inspections that could have recognized the fall from elevation hazards associated with employees utilizing warning line equipment sixteen feet above surface level without a monitor and/or conventional fall protection equipment; roofing work performed between the line and roof edge, access/egress to roof edge without warning lines path installed, and work other than roofing performed outside warning line.

Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On August 29, 2019 at 5720 Windy Drive in Stevens Point, an employee was working between a roof warning line system and the roof edge sixteen feet above surface level without utilizing a monitor and/or conventional fall protection equipment.
