29 CFR 1910.37(b)(2):  Each exit was not clearly visible and marked by a sign reading "Exit":    Water Plant:  The employer did not ensure exit signs were re-installed above all exits following painting operations in the following locations:    a) Raw Sewage Pump Room.  b) Clean Water Station #5674.  c) Spirator Room.    Notice to Citation 01, Item 003, 29 CFR 1910.37(b)(2) as required by 29 CFR 1960.8(b).  The employer failed to ensure exit signs were posted above all exits.  The employer was cited for a violation of the same standard on 05/18/2012, Inspection #311096192, 06/02/2012 of final order/settlement, and was abated by altering exit routes to a safer egress location. 29 CFR 1910.134(k)(5)(ii):  Retraining was not conducted when inadequacies in the employee knowledge or use of the respirator indicated that the employee did not retain the necessary training or skill:    Steam Plant:  The employer did not ensure re-training was conducted where employees were not sure of the type of respiratory protection required to be worn while performing job tasks such as, but not limited to refractory work, boiler maintenance and part change out operations.       Abatement Certification Required.
