29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:    a)  On or about August 22, 2012, employees were exposed to falls of greater than 6 feet while they conducted re-roofing activities without fall protection measures in place. 29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:      a)  On or about August 22, 2012, employees were exposed to falls of up to 9 feet while using a ladder which did not extend at least 3 feet above the upper landing.  The ladder was propped against a truck and did not actually reach the roof landing. 29 CFR 1926.1053(b)(16):  Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:     a)  On or about August 22, 2012, employees were exposed to falls while using a ladder which had a bent rung.
