Milwaukee St, Denver, CO 80210:  On or before September 4, 2014 the exposing employer did not ensure that employees were protected from injury, in that two employees installing shingles on a two-story residential home were not utilizing a fall protection system.  The pitch of the roof was 4-in-12.  This condition exposed employees to a fall of approximately 18 feet.
