29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Acosta Roofing, LLC at 1500 S. Milwaukee St, Denver, CO 80210: On or before September 4, 2014 the exposing employer did not ensure that employees were protected from injury, in that two employees installing shingles on a two-story residential home were not utilizing a fall protection system. The pitch of the roof was 4-in-12. This condition exposed employees to a fall of approximately 18 feet.
