29 CFR 1910.23(a)(3)(ii):  Every hatchway and chute floor opening was not guarded by a removable railing with a toeboard on not more than two sides of the opening, and fixed standard railings with toeboards on all other exposed sides.  Where operating conditions necessitated the feeding of material into the hatchway or chute opening, protection was not provided to prevent a person from falling through the opening:    (a)	Hexcel Corporation at 31815 Great Western Drive, Windsor, CO 80550:  On and before September 13, 2012, the employer did not ensure that employees were protected from falling in the following instance:   Employees working on the elevated platform portion of the Hexfit line were exposed to a hazard associated with an inadequately guarded chute floor opening through which the product came up through from the lower level.  This condition exposed employees to a floor opening in which persons could accidently walk into the opening that could result in contusions, lacerations or broken bones.    (b)	Hexcel Corporation at 31815 Great Western Drive, Windsor, CO 80550:  On and before September 13, 2012, the employer did not ensure that employees were protected from falling in the following instance:   Employees working on the elevated platform portion of the Hexfit line were exposed to a hazard associated with inadequately guarded floor openings located adjacent to the sides of the elevated equipment.  These openings had dimensions of 15.5 inches wide by 10 inches deep with a vertical metal post going through the middle.  These conditions exposed employees to floor openings in which persons could accidently walk into the opening that could result in contusions, lacerations or broken bones.

29 CFR 1910.23(c)(1):  Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railing (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    Hexcel Corporation at 31815 Great Western Drive, Windsor, CO 80550:  On and before September 13, 2012, the modified cat walk at the paper separation area on the HUD 09 Line was not guarded by a standard guardrail.  This condition exposed the employees to a fall hazard of approximately 8-9 feet during re-thread operations.

29 CFR 1910.303(b)(1):  Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    (a)	Hexcel Corporation at 31815 Great Western Drive, Windsor, CO 80550:  On and before September 13, 2012, the employer failed to provide a workplace free from recognized electrical hazards in that the electrical wires providing power to the hose heaters on the output of the mix cart had been spliced and their outer insulating sheathing was replaced with electrical tape.  This condition exposed the employees to the hazard of electrical shock.
