29 CFR 1926.350(a)(10): Oxygen cylinders in storage were not separated from fuel-gas cylinder or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour.    a) 5th floor, center of building:    An oxygen cylinder was stored adjacent to an acetylene cylinder for 5 days without use. Employees who worked on site were exposed to explosion and fire hazards. On or about 09/19/12.    Note:  Because abatement of this violation is already documented in the inspection case file, the employer need not submit certification of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.    a) 4th floor, center of building:    Employees used electric powered chop saw and screw gun to do carpentry work. The chop saw and the screw gun were connected to extension cords, which were routed to electric outlet devices on the 1st floor and were not provided with ground fault circuit interrupters. On or about 09/19/12.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM.  FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.

The chop saw and the screw gun were connected to extension cords, which were routed to electric outlet devices on the 1st floor. The extension cord that was plugged into the outlet device was missing a ground pin. On or about 09/19/12.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM. The extension cord that was plugged into the outlet device was not provided strain relief. On or about 09/19/12.
