29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: On or about September 20, 2012, at the jobsite, where the employer did not ensure construction debris such as but not limited to form and scrap lumber with protruding nails, and other debris was not scattered about the construction site. This condition exposed employees to the hazards of tripping, slipping and falling, as well as to puncture hazards from the nails.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about September 20, 2012, on steep roof, where the employer did not ensure that each employee engaged in framing activities such as but not limited to installation of roof wrap and plywood decking material on a single family home were protected from falling from an elevation of approximately 16-feet to unifinished ground below. The (CAG Construction) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315554337, citation number 1, item number 1 and was affirmed as a final order on (October 13, 2011), with respect to a workplace located at 126 Taylor Park Lp., Little Rock, Arkansas 72211.
