Dakota Ave., Denver, CO 80223: On and before 9/20/12, the employer did not ensure the rotating part on the belt drive motor and the milling station was covered to prevent accidental contact with the rotating part. This condition exposed employees to amputation hazards. 1. Dayton model #5K261K belt drive/fan motor 2. VBM-3V milling station Abatement Note: Abatement certification is not required for this item.

Dakota Ave., Denver, CO 80223: On and before 9/20/12, the employer did not ensure an adjustable guard was installed on the DoAll horizontal band saw to prevent inadvertent contact with the unused portion of the blade. (c) Tools for Bending, Inc. at 194 W. Dakota Ave., Denver, CO 80223: On and before 9/20/12, the employer did not ensure an adjustable guard was installed on the Marvel Series 8 Mark II band saw to prevent inadvertent contact with the unused portion of the blade. 29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check: a) Tools for Bending, Inc. at 194 W. Dakota Ave., Denver, CO 80223: On and before 9/20/12, the employer did not ensure all portable fire extinguishers were subjected to an annual maintenance check in that at least two extinguishers located in the shop area had not receive its annual service in over a year. This condition exposed the employees to the hazards associated with fire and smoke. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
