29 CFR 1910.106(e)(9)(iii):     Combustible waste material and residues in a building or unit operating area were not kept to a minimum, stored in covered metal receptacles and disposed of daily:    a) Tamarack Packaging Limited / 11124 Mercer Pike, Meadville, PA: On or about September 20, 2012 employees using the silk screen machines would use EasiSolv cleaner on a disposable cloth to clean screens that are in use. The EasiSolv cloths would be disposed of in a cardboard box along with other trash.

29 CFR 1910.147(c)(4)(ii):     The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this sec.    a) Tamarack Packaging Limited / 11124 Mercer Pike, Meadville, PA: On or about September 20, 2012 the control of hazardous energy procedures (lockout/tagout) did not list all steps for shutting down and securing machines and the placement, removal or transfer of lockout/tagout devices and how to verify lockout/tagout was successful for machines as well as annual audits of the lockout/tagout procedure. Machines that would need lockout/tagout include, but are not limited to, the die cutting machine, Thermoformer #94, Termoformer #85 and the Kbar Sealers.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Tamarack Packaging Limited / 11124 Mercer Pike, Meadville, PA: On or about September 20, 2012 employees were not protected from caught between injuries on the Armac Thermoformer #94 between a stationary blue support of the machine and the stationary red housing in which the plastic moves through and the top of the piston that has a metal support for the stacking portion of the machine.

29 CFR 1910.334(a)(2)(ii):     There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service:    a) Tamarack Packaging Limited / 11124 Mercer Pike, Meadville, PA: On or about September 20, 2012 a light table plug located near the sewing machines and near the silk screen cleaning area was damaged. The outer jacket pulled away from the plug end exposing electrical wires.      b) Tamarack Packaging Limited / 11124 Mercer Pike, Meadville, PA: On or about September 20, 2012 the outer jacket of an electrical cord going to an outlet box on a table next the Thermoformer #85 was pulled away from the wires at the outlet box.

The forklift data plate did contain information about use of the man lift.
