29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a.) Tamarack Packaging Ltd., 11124 Mercer Pike, Meadville, PA 16335, on or about October 4th, 2012, and times prior thereto:  Company employees were working with and around hazardous substances including but not limited to, Rubber Ink Solvent (Naphthol Spirits 66/3), and ICC 924 Liquid Haze Remover.  A written Hazard Communication Program had not been developed.

Tamarack Packaging Ltd., (Yellow flammable cabinet-Silk screening room area) 11124 Mercer Pike, Meadville, PA 16335, on or about October 4th, 2012, and times prior thereto:  A 14 ounce plastic bottle (Dawn Dish soap container), was not labeled to identify the hazard warnings of the chemical(s) contained within the container.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a.) Tamarack Packaging Ltd., (screen cleaning area) 11124 Mercer Pike, Meadville, PA 16335, on or about October 4th, 2012, and times prior thereto:  Company employees work with and around corrosive chemicals including, but not limited to, ICC 924 Liquid Haze Remover.  While a plumbed emergency eyewash and shower was available in an area adjacent to the screen cleaning area, the unit was not maintained in compliance with ANSI Z358.1-1998 for Emergency Eyewash and Shower Equipment in that:  	  1. Weekly inspections of the equipment were not conducted. The emergency eyewash unit was not activated weekly to verify proper operation.

Tamarack Packaging Ltd., (Silk screen cleaning area) 11124 Mercer Pike, Meadville, PA 16335, on or about October 4th, 2012, and times prior thereto:  Company employees used disposable dust masks (3M brand N95-Lot# 18250) voluntarily during print screen cleaning operations in the screen cleaning area. The employee that used the dust mask was not provided with the information in Appendix D to this section.
