29 CFR 1926.501(b) (13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented:                                                                           This violation occurred on September 21, 2012, at the single family home located at 400 Crestone Street in Desoto, Texas 75115, where at least three (3) employees were roofing a two-story residence without fall protection. The employees were exposed to a fall hazard of more than 20-feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the   ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder   was used to gain access:     This violation occurred on September 21, 2012, at a jobsite located at 400 Crestone Street in Desoto, Texas 75115, where the side-rails of the ladder that at least one (1) employee used to access/egress the roof did not extend at least 3-feet above the upper landing surface.
