29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by pinch points and moving parts: a. Sam line area - On or about September 21, 2012 employees on the Sam lines #2 and #3 were required to work adjacent to an inadequately guarded wire hook conveyor belt (chip board feeder). Employees were thereby exposed to hazards associated with transverse motion of a belt. b. Sam line area - On or about September 21, 2012 pinch points between the flight bar and sight guards on the Mini Sam and Sam line #3 were not guarded. Employees were thereby exposed to hazards associated with pinch points.
