29 CFR 1910.178(m)(5)(i):   When powered industrial trucks were left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, and brakes were not set    Jobsite: The operator of the telehandeler was not in the cab or in direct view of the PIT that had an employee working on the elevated platform.

29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Jobsite:  The employee who was working from a platform attached to the forks of a telehandeler was not protected from fall hazards by the use of guardrails or personal fall arrest systems.

29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)    Roof Rafters:  Employees were exposed to fall hazards of approximately 9 feet to the 3rd floor when installing the roof rafters. 29 CFR 1926.501(b)(14):   Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    Jobsite:  The employees who were working on and around window openings at approximately 23.5 inches above the floor and 14 feet above the ground below were not protected from fall hazards by the use of guardrails, safety nets or personal fall arrest systems.
