29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  Worksite: The employer did not ensure that the employees who were exposed to fall hazards while working in the residence under construction at heights above six feet were trained in fall hazards and the procedures to be followed to minimize such hazards.

29 CFR 1926.1060(a)(1)(iii): The employer shall ensure that each employee has been trained by a competent person in the proper construction, use, placement, and care in handling of all stairways and ladders:  Worksite: The employer did not ensure that the employees who were required to use the extension ladders which were in defective conditions to access both the second and third floors of the residence under construction were trained by a competent person to recognize the conditions of ladders and in the proper procedures to minimize the hazards associated with ladders.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use.  Worksite: The extension ladders, which were in defective conditions and used by employees to access both the second and third floors of the residence under construction were not inspected by a competent person prior to allowing employees to use.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  Worksite: The employer did not ensure that the employees who were working in the residential building under construction were protected from multiple fall hazards above six feet by the use of guardrails, safety nets or personal fall arrest systems.  On or about 11/30/2015, an employee fell approximately ten (10) feet to the second floor from the third floor while working in a residence under construction and sustained severe injuries from the incident.   JCH Construction, LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 653378, Citation Number 1, Item Number 3a, issued on 10/11/2012 with respect to a workplace located at 7 Carriage Lane, Riverside, CT and became a final order on 11/5/2015.
